Keeping your personal information and preferences up to date is one way to get the most out of your OptumHealth Bank HSA.

Statements
Statements for your account are posted monthly online. Log in to view your statements. Up to 18 months of prior statements are available. If you would prefer to receive statements by mail, you may fill out a Statement Delivery Change Request Form (PDF).

Contact Information
Keep your contact information up-to-date so we can reach you. We send account alerts and important information from time to time. And, our email newsletter can help you take full advantage of your HSA.

Beneficiaries
It's important to designate a beneficiary or beneficiaries and keep that information up-to-date to make sure you control where your HSA funds go if you die. If you don't designate a beneficiary, the funds will go to your legal spouse if you have one. If you are not married at the time of your death, the funds will go to your estate.

Account Closure
We hope you never want to close your account. Consider the following before you decide:

  • You don't have to close your account if you switch jobs.
  • Even if your new employer offers an HSA, you can keep and continue to contribute to your OptumHealth Bank account instead of opening a new one.
  • If you no longer have a high-deductible health plan, you can use money you've saved in your OptumHealth Bank HSA for medical expenses now and in the future – you just can't make additional contributions.

 If you do choose to close your account, fill out the Customer Request Account Closure Form (PDF).
